FTAA  application (Form No. 7-1) Location Map/Sketch map (Scale: 1:50,000 NAMRIA topographic Map) Showing Coordinates/boundaries with major environmental features/other Projects (prepared signed/sealed by Deputized GE)  Work Program Exploration Work Program (MGB Form No. 5-4) Environmental Work Program (MGB Form No. 16-1 or 16-1A) Certificate of Satisfactory Environmental Management/Community Relations Record/Certificate of Exemption/Application for CEMCRR Proof of Technical Competence including track records in mining operations/environmental management, bio data Proof of financial capability Latest Audited Financial Statement Annual Report for the Preceding Year Certified Photocopy of SEC Registration Articles of Incorporation and By-Laws Minimum Authorized Capital Stock of P10 Million Minimum Paid up Capital of P2.5 Million Credit Line/Bank Guarantees/Deposit Declaration of Total Area covered by Approved/pending Mining Applications Affidavit of Undertaking Proof of payment of filing and PD 1856 fees Corporate Secretary’s Certificate Authorizing the Corporation’s Representative to file/sign and/or transact with the Bureau concerning the MPSA proposal
